Mum has had her assessment. What happens now, and how do we get the care she needs?

It’s completely understandable to wonder what happens next after your mum’s assessment. This is a big step, and knowing how to move forward can help you and your family feel more confident. Here’s a guide to what happens after the assessment, depending on your mum’s situation.

What Happens After the Assessment?

  1. You Receive the Support Plan During the Assessment
    • If your mum was assessed for services under the Commonwealth Home Support Programme (CHSP), she may receive her support plan right there and then.
    • The assessor will let you know what services she’s eligible for and help you understand how to access them.
    • Next Steps:
      • Connect with aged care providers and start arranging services.
      • Learn more by visiting the CHSP Assessment Outcome page.

  2. You’re Waiting for the Support Plan and Result
    • If your mum’s assessment was for a Home Care Package, short-term care, or an aged care home, the results need to be reviewed first.
    • The support plan and result will typically arrive within 2 to 6 weeks by post, email, or in her My Aged Care Online Account.
    • What to Do:
      • Discuss with the assessor how you’d like to receive the results.
      • Call My Aged Care on 1800 200 422 if you have any concerns.

  3. You’ve Received the Support Plan and Results
    • Your mum’s support plan will include detailed information on what services she can access.
    • If she’s not eligible for government-funded services, the plan will suggest other care options.
    • Next Steps:
      • Follow the guidance based on the program listed in her support plan:
        • Commonwealth Home Support Programme (CHSP)
        • Home Care Packages
        • Short-Term Restorative Care
        • Residential Respite Care
        • Transition Care
        • Aged Care Homes (Residential Care)

      • You can also view the support plan in the My Aged Care Online Account via myGov.

Need Help Navigating This Process?

  • Check Progress: Log into the My Aged Care Online Account or call 1800 200 422 to track your mum’s application or assessment.
  • Wait Times: For Home Care Packages, the availability depends on national wait times and local service providers. Call My Aged Care for help understanding these wait times.
  • Fee-Paying Services: If you prefer additional support, there are fee-paying aged care navigation services available. Make sure to research these carefully to understand their costs and benefits.

Have Concerns or Feedback?

If something doesn’t feel right or you disagree with the assessment outcome, you can provide feedback or make a complaint through the My Aged Care complaints page. Your voice matters, and there are processes to ensure your concerns are heard.

This journey may seem overwhelming, but you’re not alone. Knowing the next steps and available support can make this process clearer and help your mum get the care she needs.
